While John Dutton did have cancer at one point, the storyline surrounding his illness is a tad confusing. John’s illness was definitely introduced to apply more pressure to the ranch’s successor plot, all while adding more uncertainty to the fate of the land and family legacy. In the Yellowstone season 2 premiere, John is convinced his cancer is back when he suddenly collapses. After being rushed to a ranch veterinarian, John coughs up blood and explains he had colon cancer, which he fears is back with a vengeance. When the vet realizes John is suffering from a ruptured ulcer, he performs emergency surgery and cauterizes the wound on the spot.

The ruptured ulcer moment explains why John Dutton wasn’t looking too good toward the end of Yellowstone’s first season. Still, John did have cancer originally, which means that story could resurface in Yellowstone season 5, part 2. In part 1, John doesn’t have cancer — though that was well before Costner’s departure was cemented. Now, it’s hard to imagine a truly satisfying way for Costner to leave Yellowstone: an off-screen death would feel cheap, but having John leave the ranch behind is equally ill-fitting.
